Risk Management: NJ Department of Health, Reporting Serious Preventable Adverse Events

An adverse event that is preventable and results in patient death, loss of body part, disability, or loss of bodily function lasting more than seven days or still present at time of discharge.

Examples:

  • Retained foreign object
  • Suicide attempt
  • Pressure Ulcers
  • Falls w/injury

Serious preventable adverse events must be reported to the DOH within 5 days. The Hospital has 45 days to complete a Root Cause Analysis (RCA) and submit the findings to NJDOH.
